The goal is to determine whether saliva can validate a test to determine a child's risk for developing cavities. Your child simply needs to "spit" into a tube. If your child is eligible to take part in this study, they will receive a $15 gift certificate to Toy's R Us -- $15 just for spitting.</p> <h2>Who is Eligible?</h2> <p>If your child is between the ages of 6 and 17 years old and is covered by Northeast Delta Dental insurance, they can participate in the research. Everything is kept anonymous, but parents must consent.</p> <h2>To Participate</h2> <p>Click on the link below to Northeast Delta's website, go to <u>Download Forms</u> (in the left-hand column), and download the forms under the <u>Care Test Research Project</u>. Click on the appropriate form based upon your child's age. Please fill these forms out completely and bring them to your appointment at Amherst Pediatric Dental Associates.</p> <p align="center"><a href="https://www.eligibility.nedelta.com/" target="new">https://www.eligibility.nedelta.com/</a></p> <p>Be sure that you have downloaded the <b>Parental Consent</b>, <b>Assent Form</b> (for kids 7 and older), <b>Medical History Questionnaire</b>, and <b>Collection Information</b>. If you choose you can also view the Directions for Collection which will show you pictures of how the test is done.
